In 2015, Aunt Martha’s served more than 3,500 Township residents, most of whom received some combination of primary care, dental, mental healthor women’s health services at one of the organization’s health clinics. Now, it will work directly with the Township and local stakeholders to make sure that residents are connected with and receive the assistance or services they need.

“Community Connections is a gateway, and more importantly, a guide. We’re going to understand your unique situation. We’re going to do everything we can to find and connect you to the help you need.”

Aunt Martha’s Community Connections program is located at 15350 S. Oak Park Ave., Oak Forest, IL 60452. The office is open Monday through Friday, from 9 a.m. to 5 p.m., and can be reached by calling a 24-Hour Crisis Line at (708) 679-8100.
